Destination Rehab is a 501(c)3 non-profit physical therapy practice founded in 2016 in Bend, Oregon to empower adults with disabilities to engage in the outdoors and their local community. They offer outdoor-based physical therapy and neurorehabilitation for people with chronic medical and neurologic conditions such as brain injury, stroke, Parkinson‘s, multiple sclerosis, spinal cord injury, cerebral palsy, post-concussion, and vestibular disorders. Our team is passionate about helping everyone access outdoor adventure by providing specialized training and small group Adventure opportunities. Facilitated by Physical Therapists with neurologic-specific training, all activities and outings have an evidence-based approach and our mission is to promote improved community participation, confidence, and quality of life through functional skills training and adventure challenges. Participants are given tasks based on their individual abilities, interests, and goals to promote their own problem-solving and independence. They benefit from our small group environment and from the encouragement and support of their peers, the program staff, and our volunteers. Destination Rehab wants our participants to experience new adventures and challenge themselves to push beyond what they thought was possible, to rediscover a favorite activity, and to build a community of mentorship and support that improves their independence, participation, and quality of life.
